USCIS immigration physical (Immigration medical) exam

The United States Citizenship and Immigration Services (USCIS) requires a medical and physical exam as part of the residency and/or entry process to the United States. The rules and requirements are designated by the USCIS:

Generally, all applicants filing for adjustment of status to that of a lawful permanent resident must submit Form I-693 completed by a designated civil surgeon. Form I-693 is used to report results of a medical examination to USCIS. The examination is required to establish that an applicant is not inadmissible to the United States on public health grounds.

To speed up the process, please provide all historical vaccination and immunization records that you may have to Dr Cvengros or Dr Munoz.

Immigration Physical Exams

Our exams are done by USCIS designated civil surgeons (bi-lingual in Spanish). We offer clear, transparent pricing and offer immunizations on-site for convenience. 

Your Immigration exam includes the following in a Sealed envelope containing the USCIS I-693 form. A copy of the USCIS I-693 form and lab test results for your records will alsobe provided.

Tuberculosis (TB) blood test.
For applicants of age 2 and older

Prepare for the I-693 Exam

To avoid delays, please review the items below. Text or Call us at 773-755-2600 if you have any questions.

  1. Bring your passport, driver’s license or any other government issued ID. Ensure all documents are unexpired.
  2. Bring your entire vaccination record.
  3. If you recently got a flu shot, kindly bring those records.
  4. Effective 10/01/2021, COVID-19 vaccination will be required.
  5. We have the USCIS I-693 forms at the office for your convenience.

How does it work?

Before your visit

Please bring the following items with you to your appointment:

  1. Passport or Government-issued Identification. All identification documents must be unexpired
  2. Medical / Vaccine records
  3. Arrive early to your appointment to complete the I-693 form at our office

During your visit

Briefly, what will take place:

  • Detailed medical history and physical exam by Dr Cvengros or Dr Munoz including the review of required vaccinations. If vaccines are required, we can provide them for an additional charge.
  • Tuberculosis (TB) blood test (Age 2 and older)
  • Blood test for syphilis (Age 18-44)
  • Urine test for gonorrhea (Age 18-24)

Follow up or Pick up completed paperwork

Once the lab results are back, you will be contacted by our staff, informed of your test results, and instructed to pick up the completed forms at our office:

The USCIS medical exam and rules are to ensure that anyone who comes to live in the United States is healthy and has all the required vaccines. Our staff are bi-lingual and certified by USCIS to perform the immigration medical exams.

Yes! Dr. Teresa Cvengros & Dr. Gilberto Munoz are both designated civil surgeons by the USCIS.

We utilize Quest Labs for their speed and accuracy. In general, the results are ready within 48-72 hours. 

No. Your insurance does not cover Immigration medical physicals. 

If you require vaccines, we can provide you with a itemized receipt to get get reimbursed for covered immunizations from your insurance.

We do our best to expedite your medical exam. However, there are some external resources outside our control that could take 48-72 hours to return.

In general, we are able to perform your physical, blood and urine tests, and complete the forms ready to pickup in 3-4 days.

We suggest planning for the entire process to take 4-5 days prior to your appointment with your attorney or USCIS officials.
