We will verify your benefits with your insurance company. We do this so that you will have an estimate of your financial responsibility. At times, you may owe Co-Pay, Deductibles, or Co-Insurance. You may also receive a bill from us for any remaining balances.

Please keep in mind, you are responsible for knowing details about your insurance such as your annual deductible amounts and co-pay. Kindly bring a copy of your insurance card with you to each visit.
